Our Leadership


Sabrina Hogan

President of EleHealth

Sabrina Hogan joined Blue Cross and Blue Shield of Louisiana in 2019 to, among other roles, provide executive leadership for the development of its new subsidiary, EleHealth.

The purpose of this new Louisiana-based health care company is to help providers across the state improve the delivery and quality of more affordable health care through population health services. With 30 years of experience providing strategic leadership and driving continuous quality improvement in health care organizations, Hogan was a natural choice to lead EleHealth.

Hogan’s experience includes overseeing all direct care delivery programs and services for Blue Cross. She worked closely with the company’s provider assets and Clinical Operations division to apply a continuous-improvement approach to boost clinical quality, patient safety and patient satisfaction. This included in-house clinical programs and services, as well as work with network provider partners. Hogan helped Blue Cross leadership identify new business opportunities and implement best practices into our patient-facing programs.

Before that, Hogan’s experience included providing strategic leadership and driving continuous quality improvement in health care organizations. She came to Blue Cross from St. Francis Medical Group and St. Francis Medical Center in Monroe, where she held a variety of leadership roles. Most recently, Hogan was chief operating officer of the St. Francis Medical Group and vice president for medical staff services and health information management (HIM). In that role, she steered operational strategy and was responsible for recruiting, onboarding, credentialing and overseeing performance management for more than 50 multi-level health care providers. During Hogan’s time at St. Francis, the organization received Top Tier Distinction for Quality Performance from the Blue Cross and Blue Shield Association. She has also provided consulting services for patient access programs, clinical pathway development and case management redesign.

Hogan earned a Bachelor of Science degree in nursing and a master’s degree in nursing with a concentration in healthcare systems management from Loyola University in New Orleans. She was a National Baldrige Examiner from 2008-2010. This is a select group of professionals who evaluate applicants for the Malcolm Baldrige National Quality Award, which is the only Presidential award recognizing organizational performance excellence.


Kristi Jones, RN, CPHQ

Director, Population Health Services & Quality

Kristi Jones is a seasoned healthcare executive with over 25 years of experience spanning clinical nursing, quality improvement, and population health leadership. She currently serves as Director of Population Health and Quality at eleHealth, a subsidiary of Blue Cross Blue Shield of Louisiana, where she leads strategic initiatives to enhance clinical outcomes and reduce costs across affiliated practices. Kristi oversees a multidisciplinary team of clinicians and support staff, driving quality improvement programs and data management for both Network and Medicare ACOs.

Prior to joining eleHealth, Kristi held leadership roles at Affinity Health Group and Vantage Health Plan, where she designed and implemented value-based care programs, managed CMS and NCQA compliance, chaired oversight committees for MIPS and CPC+, and led “early adopter recognitions” for the only recognized Patient-Centered Connected Care practices in the State of Louisiana. Her expertise in continuous quality improvement, care gap closure strategies, and EMR reporting systems has positioned her as a trusted advisor to executive teams and physician champions.

Kristi began her career as a nurse in various clinical settings including allergy, hemodialysis, and OB/GYN, and has consistently advanced through roles of increasing responsibility. She holds an Associate of Science in Nursing from Regents College of New York and is certified in healthcare quality (CPHQ) and Lean Six Sigma Green Belt.

Her leadership is defined by a commitment to excellence, innovation in care delivery, and a deep understanding of the evolving healthcare landscape.


Sherri Joubert

Lead Operational Reporting & Data Analyst

Sherri Joubert serves as Lead Operational Reporting & Data Analyst for EleHealth. In this role, Sherri provides data and analytical support to all stakeholders of the organization through leveraging utilization and financial data from various sources. She is focused on providing meaningful reporting and data driven insights that deliver value and drive innovation.

Sherri previously served as Medicare Clinical Quality Program Manager at Anthem, Inc. where she helped expand value-based program participation in the Louisiana DSNP market and analyzed clinical data to assist in the development of quality improvement activities. During her career of more than 10 years at Blue Cross and Blue Shield of Louisiana, Sherri has developed a wide range of experience in Claims Operations, Audit and Value Based Care. Among roles served at BCBSLA, she most recently served as Manager, Accountable Care. In this role, she helped lead a team of analysts who delivered actionable information to providers to support the execution of the BCBSLA Value-Based Shared Savings Program.

Sherri earned a Bachelor of Science in Information Systems and Master’s in Business Administration from Louisiana State University. She is a certified Lean Six Sigma Black Belt, Certified Information Systems Auditor and is also designated as a Certified Healthcare Consultant and Professional, Academy for Health Care Management.


Lisa Nations

Director of Finance

Lisa Nations serves as Director of Finance for EleHealth.  In this role, Lisa provides financial leadership and manages the financial operations of EleHealth.  She is also responsible for financial planning, analysis and reporting and is focused on ensuring effective financial management and reporting for EleHealth. 

Lisa has 20 years of experience in both hospital and clinic financial operations, having most recently served as Director of Finance for the Franciscan Missionaries of Our Lady Health System in Monroe, which includes St. Francis Medical Center, St. Francis Medical Group and St. Francis Foundation.  She has a broad array of experience ranging from revenue cycle and payor reimbursement to more traditional accounting and finance functions. 

Lisa is a highly adaptive and process-driven healthcare financial professional providing reliable financial and analytical support to all key organizational stakeholders.  She works collaboratively across interdepartmental teams, leadership teams and other organizational stakeholders to accomplish operational and strategic goals.  Lisa earned a Bachelor of Business Administration in Finance and a Master of Business Administration from the University of Louisiana at Monroe.
