Jasmine Warren, APRN, FNP-C

Originally from New Orleans, Louisiana, Jasmine began her nursing education at the historic Charity School of Nursing. She continued advancing her clinical expertise by earning a Master of Science in Nursing with a specialization in Care Coordination from Capella University and later completed her Post-Graduate Family Nurse Practitioner Certificate through South College. Jasmine is board-certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners.

As a new Family Nurse Practitioner, Jasmine brings enthusiasm, compassion, and a strong commitment to patient-centered care. Her primary areas of interest include Primary Care and Hospice, where she is passionate about supporting patients and families through all stages of their healthcare journey.

At EleHealth Care Connect, Jasmine enjoys the opportunity to help bridge gaps in healthcare while fulfilling her lifelong dream of serving as a healthcare provider. She is dedicated to educating and empowering individuals to take an active role in their health and wellness while complementing the care they receive from their primary care providers. One of the most rewarding aspects of her role is helping patients gain a comprehensive understanding of their overall health and well-being. Jasmine also values the welcoming, supportive, and collaborative culture at EleHealth Care Connect, where team members work together to provide exceptional care and encourage one another’s success.

Outside of her professional life, Jasmine enjoys spending quality time with her daughter and creating meaningful memories together. Whether crafting, dancing, singing, cooking, reading, watching movies, or enjoying the outdoors, they share a love for creativity, laughter, and helping others. These experiences reflect the compassion, positivity, and dedication that Jasmine brings to both her personal life and her work as a healthcare provider.
